PROGRAM DESCRIPTION: Primary immunoglobulin deficiency disorders (PIDD) represent a broad spectrum of diseases that can lead to more frequent and more severe infections of the upper airway, mainly chronic rhinosinusitis (CRS) or recurrent acute rhinosinusitis (RARS). PIDD are classified into the following conditions: Common variable immunoglobulin deficiency (CVID), Specific antibody deficiency (SAD), IgG subclass deficiency, selective IgA deficiency. There is clear evidence that prevalence of PIDD is higher in patients with CRS than in the general population. However, many questions remain unanswered. These include what defines difficult-to-treat CRS and when is an evaluation for PIDD warranted in these patients. What is the role of the otolaryngologist in evaluation of PIDD and when is referral to a clinical immunologist warranted? What is the different treatment options for patient with PIDD and CRS?

OUTCOME OBJECTIVE 1: Discuss the evaluation and diagnostic workup of primary immunoglobulin deficiency disorders (PIDD) in patient with rhinosinusitis.

OUTCOME OBJECTIVE 2: Discuss the role of vaccination, prophylaxis antibiotics, and immunoglobulin replacement therapy in patients with PIDD and rhinosinusitis.

OUTCOME OBJECTIVE 3: Discuss the role of endoscopic sinus surgery in patients with PIDD and rhinosinusitis.

BACKGROUND STATEMENT: The role of primary immunoglobulin deficiency disorders (PIDD) in difficult-to-treat CRS has been demonstrated in prior studies. The prevalence of PIDD is higher in patients with CRS than in the general population. It is important to consider these diseases when dealing with CRS patients.
