PROGRAM DESCRIPTION: Rhinoplasty is one of the most popular surgeries in the world and the third most common plastic surgery procedure in the United States with approximately 200,000 completed per year. Rhinoplasty is performed for functional and cosmetic purposes, and is a complex procedure adjusted based on a patient’s goals and nasal anatomy.

Nasal osteotomy is one of the key steps in rhinoplasty, frequently performed to alter the width of the bony nasal period, straighten a deviated nose, or close an open roof deformity. Nasal osteotomies are not only technically challenging to perform but can be associated with post-operative complications such as nasal obstruction from excessive narrowing, hemorrhage, asymmetry, and aesthetic deformity. A study published in 2019 in Plastic and Reconstructive Surgery established nasal osteotomy as one of the steps in rhinoplasty that residents find most difficult and have the least confidence in performing. The same study identified a hands-on simulation as a method to improve competency.

At our institution, we have developed a low cost, 3D printed nasal osteotomy model to allow residents to practice this procedure and gain tactile, interactive experience. In this simulation session, the trainees will first review the relevant anatomy and indications for nasal osteotomy. After discussing the risks and benefits of medial and lateral nasal osteotomies, the correct approach and technique will be demonstrated. Trainees will have the opportunity to practice medial and lateral osteotomies on a 3D model and receive feedback from the presenters.

BACKGROUND STATEMENT: The technically challenging nature of nasal osteotomy during rhinoplasty can limit residents and trainees in their ability to achieve proficiency. This training utilizes a 3D model to provide a hands-on simulation experience for nasal osteotomy.
