PROGRAM DESCRIPTION: As otolaryngologists, we often use antibiotics to manage difficult chronic infections and prevent infections after surgery. Systemic antibiotics are a powerful tool for treating common head and neck complaints, however, the downsides of these drugs are often under-recognized. This gap in knowledge contributes to over-prescribing of antibiotics in situations where alternative therapies would be more effective. Antimicrobial overuse leads to individual patient complications, growing resistance patterns and increasing spread of difficult-to-treat infections. This problem is a major public health concern, and both the WHO and CDC have developed global action plans to fight inappropriate antibiotic use. The expert panel will address key areas of maximizing antimicrobial therapies in practice. We will cover evidence based guidelines on surgical prophylaxis and look at alternative therapies and the development of resistance to topical antimicrobials in chronic bacterial diseases using chronic draining ears as an model Risks of overuse of antibiotics will be reviewed. Additionally, panelists will present alternative treatment options to standard systemic antibiotics, including cutting-edge research into developing microbiome-based treatment options for common head and neck infections. The goal of this interactive lecture is to improve knowledge of evidence-based best-practice of perioperative antibiotics and give surgeons practical and effective options for treating chronic infections without long-term systemic antibiotics. Additionally, alternative antimicrobial therapies that can be used to avoid antibiotic treatments will be discussed.

OUTCOME OBJECTIVE 1: Implement evidence-based use of perioperative antibiotics into individual practice--using the correct antibiotic at the correct time

OUTCOME OBJECTIVE 2: Examine the use of antibiotics in chronic infections--evaluating their benefits, risks and failures in order to improve individual practice in treating chronic infections

OUTCOME OBJECTIVE 3: Analyze current research in alternative antimicrobial therapies, and apply treatments with good evidence to current clinical practice

BACKGROUND STATEMENT: This timely topic applies to all practicing otolaryngologists and represents a challenging clinical dilemma. The diverse panel has personal and research interests in the topic, and represents an international perspective.
