Animated Videos as a Tool for Teaching Uncommon ENT Diagnoses

Introduction: Nasal septal hematomas (NSH) are uncommon entities seen in Emergency Rooms. First-line care providers need to maintain a high index of suspicion for NSH in patients with nasal trauma. Can we increase their understanding of the appropriate management of NSH with a short cartoon video?

Methods: A convenience sample of Emergency Medicine attendings, fellows, residents, advanced practice providers (APPs), and medical students (MS) were recruited via email to do an 8-question pre-test, watch a 5-minute video produced by the research team, and do a 7-question post-test. This included 6 questions centered on key facts in the emergency management of NSH, and a 10-point Likert scale depicting their comfort managing NSH. Higher scores indicated higher levels of comfort. The video was a low-cost cartoon made without a media expert's contribution.

Results: 131 participants completed the study, 62 (47.3%) were attendings, 32 (24.4%) were fellows or residents, 11 (8.4%) were APPs, and 26 (19.8%) were MS. Thirty-three (25.2%) had diagnosed a NSH already, 26 (78.9%) of whom were attendings. There was a significant improvement in the knowledge scores, with a mean pre-test score of 80.9% (95% CI 77.5-84.3) and a post-test score of 90.7% (95% CI 88.2-93.3, p < .001). This was true for each subgroup based on current position, and both pre-test and post-test scores were significantly different among the subgroups, with the highest scores for the attendings and APPs. Initial comfort managing NSH ranged from 4.37 for attendings to 1.65 for MS (mean 3.27, 95% CI 2.95-3.58, p < .001). Post-test mean comfort level ranged from 5.4 for attendings to 3.9 for MS (mean 4.80, 95% CI 4.56-5.05, p < .001). The increase in comfort scores for the whole group was statistically significant (p < .001).

Conclusions: Our short video increased the comfort of management of NSH for providers involved in Emergency Medicine regardless of their level of experience. This modality may be a viable way of teaching specific otolaryngology topics to medical practitioners.
