Introduction: Oropharyngeal dysphagia is frequently observed after radiotherapy in head and neck cancer patients. Double balloon dilation effectively addresses pharyngoesophageal segment (PES) stenosis within this population. Cadaveric models suggest that the PES is structurally similar to a kidney bean. The thyroid cartilage differs based on biometric measures and may affect PES distension due to anatomical proximity. These factors may contribute to the variability in expansion of the PES therefore diversifying its maximum dilation potential. Understanding anatomical PES variants are important in enhancing pre-operative planning and therapeutic management.

Methods: A retrospective review of patients’ maximum dilation potential who underwent double balloon dilation of the PES was performed (2020-2023). Biometric measurements (sex, height, and weight) were collected. Analysis included logistical regression and one-way ANOVA.

Results: Of 112 patients, 22.3% were female with an average age of 66.3 years ± 9.7. The average height of this population was (174.4 cm ± 15.5) and weight (76.1 kg/m2 ± 15.5). The average overall dilation size was 31.0 mm. The average male dilation size was 31.6 mm ± 4.62 and the average female 28.8 mm ± 5.64. Based on height, weight, and BMI there was no significant difference in dilation size (p >0.05). A statistically significant difference (p=0.02) was observed between PES dilation size and sex.

Conclusions: Our analysis suggests a patient’s sex may play a significant role in determining PES dilation size whereas that height and weight may be less predictive. Biometric measurements may aid in developing a nomogram to estimate the size of the PES.
