PROGRAM DESCRIPTION: Management of chronic rhinosinusitis (CRS) refractory to appropriate medical and surgical intervention continues to remain a challenge. However, recent years have witnessed significant expansion and innovation in our treatment armamentarium for such recalcitrant disease. The purpose of this panel presentation is to discuss the diverse spectrum of therapies currently available for refractory CRS, with an emphasis on clinical outcomes and the relevant evidence. Emerging systemic biologics and their role in the treatment of sinonasal disease will be addressed. The mechanism of action, immunomodulatory capabilities, and clinical efficacy of these burgeoning agents for various CRS subtypes will be elucidated. The vast array of topical anti-infective and anti-inflammatory agents will be discussed, including drug-eluting implants and a review of the corresponding evidence. Novel endoscopic surgical approaches for management of recalcitrant CRS will be delineated. The indications and techniques involved with such revision surgical salvage procedures will be described in conjunction with video presentations. In addition, recent technologic innovations and the current evidence for their use in refractory CRS will be presented, including radiofrequency and cryoablation therapy. This session will be conducted by academic rhinologists using a multimedia, interactive case-based format with audience polling.

OUTCOME OBJECTIVE 1: Discuss the role of systemic biologics, emerging immunomodulators, and topical additive therapies in the treatment of refractory chronic rhinosinusitis

OUTCOME OBJECTIVE 2: Describe novel revision endoscopic surgical procedures for management of recalcitrant chronic rhinosinusitis; including indications, approaches, and recently introduced technologic innovations

OUTCOME OBJECTIVE 3: Understand the current evidence, clinical efficacy, and optimal application of each therapeutic modality in patients with refractory chronic rhinosinusitis

BACKGROUND STATEMENT: Need to discuss the diverse array of medical/surgical treatment options and understand the relevant evidence for refractory CRS, which continues to be a challenge for many otolaryngologists to manage. Three gap topics will be covered including biologics, revision surgery, and rhinorrhea.
