(051) Association Between BMI and Nasal Polyposis Chronic Rhinosinusitis Patients

Introduction: Obesity is an ongoing pandemic that increases the severity and risk of multiple health conditions, including asthma. Given the overlap in the inflammation between asthma and chronic rhinosinusitis (CRS), our objective in this study was to assess the impact of overweight/obesity on medical comorbidities in patients with CRS.

Methods: We performed a retrospective study on adult patients who were diagnosed with CRS at a single institution between 2020-2022. We compared the incidence of comorbidities and clinical factors for CRS patients, grouped as lean (BMI < 25 kg/m2) and overweight/obese (BMI ≥ 25 kg/m2). The comorbidities and clinical factors included were allergic rhinitis, sleep apnea, asthma, COPD, hyperlipidemia, hypertension, diabetes, gastroesophageal reflux disease (GERD), nasal polyposis, the number of sinus surgeries performed, and radiologic severity (Lund-Mackay scores).

Results: A total of 1,165 patients were included in our cohort. The population had a median age of 57 (range 18-94). 714 (61.29%) patients were lean, and 451 (38.71%) patients were overweight/obese. In patients with CRS, hypertension, sleep apnea, and nasal polyps were independently associated with overweight/obesity with increased odds compared to their lean counterparts (hypertension: OR= 2.29, 95% CI [1.72, 3.06], p-value < 0.0001; sleep apnea: OR= 2.85, 95% CI [1.97, 4.13], p-value < 0.0001; nasal polyps: OR= 1.39, 95% CI [1.04, 1.87], p-value= 0.028).

Conclusions: Our retrospective study showed an increased odds of having sleep apnea, hypertension, and nasal polyps in overweight/obese patients with CRS compared to their lean counterparts. Further research is needed to evaluate this association mechanistically.
