Introduction: Eosinophilic otitis media is characterized by a viscous middle ear effusion, with predominance of eosinophils, tympanic perforation, refractory otorrhea to conventional management. Consequently, it results in conductive or mixed hearing loss, associated with asthma and/or nasal polyposis. Hence, treatment is controversial. We report the case of a patient with eosinophilic otitis media and a history of multiple surgical ear procedures who was successfully managed with Dupilumab.

Methods: This case report discusses a patient suffering from Eosinophilic otitis media with moderate mixed hearing loss. He was treated between November 2021 and August 2022 by the Otorhinolaryngology service of San Jose children´s hospital. He received biological therapy followed by bilateral timpanoplasty.

Results: At one year follow up there was an imorvement in SNOT-22 scores, radiological and audiological parameters and eosinophil infiltration in middle ear mucosa.

Conclusions: Based on the sustained improvement in clinical, audiological, immunological, and radiological characteristics obtained in this case report, we suggest Dupilumab therapy in patients eosinophilic otitis media who have not responded to medical or surgical interventions.
