(131) Demographic Factors Impacting Odds of Inpatient Admission for Angioedema

Introduction: This study evaluates patient demographic factors that influence inpatient admission rates among emergency department cases of angioedema. While angioedema disposition principally depends on airway and respiratory stability, we hypothesize that there are demographic trends underlying admission patterns.

Methods: Data was extracted from the National Emergency Department Sample (NEDS) for patients diagnosed with angioedema in 2015, 2017, and 2019. Results were extrapolated to the entire US emergency department population using established sampling weights. Odds of inpatient admission for the national angioedema population were generated with multivariate logistic regression. P-values < 0.05 were considered statistically significant.

Results: A total of 242,315 ED visits were included in this study. Statistically significant trends were identified across all demographic variables. Females (95% CI [.87, .97], P < .002), patients less than 10-years-old (ref.), self-pay (95% CI [.54, .71], P < .0005) and private insurance patients (95% CI [.65, .79], P < .0005), non-metropolitan hospitals (95% CI [.35, .51], P < .0005), and non-trauma facilities (ref.) experienced the lowest admission odds. Demographics associated with increased admission odds include patients aged 45 to 64 (95% CI [5.50, 10.38], P < .0005), patients from zip codes with a median household income in the lowest quartile (ref.), metropolitan teaching hospitals (95% CI [1.13, 1.44], P < .0005), and Level I trauma centers (95% CI [1.30, 1.83], P < .0005). Increased scores on the Charles Comorbidity Index also confer greater odds of admission, especially for patients scoring 3+ (95% CI [9.43, 11.54], P < .0005).

Conclusions: These results highlight demographic-level trends in angioedema management. Further research is warranted to determine the cause underlying these differences.
