(159) Effect of Neuropsychiatric Comorbidities of Hospital Outcomes in Pediatric Cholesteatomas

Introduction: Cholesteatoma care for pediatric patients with neuropsychiatric (NP) disorders often poses various diagnostic and treatment challenges for treating otolaryngologists. The aim of this study is to better define the role that these disorders have on inpatient hospitalizations related to cholesteatoma diagnoses.

Methods: The Kids’ Inpatient Database (KID) 2003-2019 was queried for patients with cholesteatomas. Patients were stratified into two groups based on presence of comorbid NP diagnosis (developmental disorder, Down syndrome, intellectual disability, behavioral disorder, schizophrenia, other psychotic disorder, mood disorder, and anxiety). Univariate and multivariate analyses were performed to compare patient characteristics and outcomes between the two groups.

Results: A total of 2,288 cases were identified. Of these, 605 (26.5%) patients had a NP diagnosis. The median age of patients in the NP group was 10 years. Caucasian (N=1168, 51.0%), and male (N=1428, 62.4%). Patients with NP diagnoses were found to have a significantly longer length of stay (3.13±0.113 vs 5.14±0.417, P< 0.001), increased mean hospitalization charges ($39,757.06±$1,684.36 vs $53,180.92±$9,034.58, P=0.001). However, patients with NP diagnoses did not have an increased rate of hospital complications.

Conclusions: The results from our study showed that patients with comorbid NP conditions did not have any increase in hospital related complications. However, they did have longer lengths of stay and increased hospital charges. These findings may be related to more challenging diagnostic evaluation due to patient cooperation and anatomic variants. These findings can offer evidence-based guidance to providers when counseling family members about treatment course and surgical planning.
