Introduction: We aimed to identify racial and ethnic disparities in perioperative outcomes for pediatric patients undergoing surgery for head and neck masses using the pediatric National Surgical Quality Improvement Program (NSQIP-P).

Methods: The NSQIP-P database was reviewed for pediatric patients who underwent surgery for a head and neck mass from 2015 to 2020 according to ICD-10-CM diagnostic codes. Variables looked at were patient demographics, comorbidities, operative time and location, complications, length of stay, readmission, and unplanned reoperation.

Results: A total of 4647 children (124 Asian, 604 Black, 968 Hispanic, 2218 White, and 733 Other Race) were identified with a mean age of 8.1 years (SD: 5.3) at time of surgery. The most common diagnoses were congenital malformations of other endocrine glands (73%) and malignant neoplasms of thyroid and other endocrine glands (18%). Asian patients were least likely to have current or prior history of malignancy (68% vs. 79-89%, P < 0.001), and Black patients were significantly more likely to receive outpatient treatment (P = 0.009). Operation time and time under anesthesia were significantly different among patient groups (P < 0.001) with Asian patients experiencing the longest mean times at 140 and 192 minutes respectively. Complication rates did not significantly differ among groups. On multivariate analysis, ASA class, malignancy history, and structural pulmonary abnormalities were significantly associated with length of hospital stay greater than 2 days, readmission, and reoperation. When adjusting for comorbidities, race/ethnicity did not significantly affect postoperative outcomes.

Conclusions: Disparities in length of hospital stay, readmission, and reoperation related to race and ethnicity were not identified. Comorbidities such as ASA class, history of malignancy, and structural pulmonary abnormalities have a greater impact on postoperative outcomes. Further research is warranted to better understand driving factors for achieving equitable outcomes in pediatric head and neck mass surgery.
