(441) Screening Children with Cleft Lip and Palate for Sleep Apnea

Introduction: The Pediatric Sleep Questionnaire (PSQ) is a validated tool for screening children for obstructive sleep apnea (OSA), but its role in screening children with cleft lip and palate (CLP) has not been well studied. This study aims to determine how the PSQ can guide further workup and management of OSA in children with CLP.

Methods: Retrospective chart review from 2014-2018 of patients with cleft lip and/or palate who are followed in a multidisciplinary clinic at a tertiary care institution. Each participant was evaluated with the PSQ. Data were collected on past medical and surgical history, physical findings, and outcome after PSQ screening.

Results: Five hundred and fifty-nine patients were screened through CLP clinic, and 28% were syndromic. Overall mean PSQ score was 4.2. Seventeen patients (3.0%) were referred for polysomnograms (PSG); PSQ score was significantly higher for those children referred for PSG (10.8 vs. 3.9, p< 0.001). Of those referred for PSG, 7/17 had OSA as defined as AHI >1, with an average PSQ score of 10.7. There was no significant association between PSQ score >8 and AHI >1 (p=0.74).

Conclusions: For children with CLP, a higher PSQ score was significantly associated with recommendation for PSG. However, there was no significant association between a higher PSQ score and AHI >1. Further analysis will investigate the association of individual PSQ questions as predictors of further workup and/or intervention for OSA.
