Introduction: Our study aims to elucidate how epidemiological factors, including race, affect advanced stage diagnosis of oropharyngeal cancer (OPC) at presentation.

Methods: We analyzed data on OPC cases from The Florida Cancer Data System (FCDS), a statewide cancer registry, diagnosed between 2010-2017. We gathered data on patients’ age at diagnosis, sex, race, ethnicity, marital status, cigarette smoking status, residential location, primary insurance status, and SEER disease stage (regional or distant) at diagnosis. Sociodemographic factors among black and white OPC patients were compared using chi-square test. Logistic regression analysis was performed to determine risk factors associated with advanced disease stage at diagnosis.

Results: Among 8,459 OPC cases, 506 (6%) were black, 777 (9%) Hispanic, 4946 (59%) < 65, 6994 (83%) male, 1101 (87%) diagnosed advanced stage, 2156 (26%) current cigarette smokers, 3223 (38.1%) had private insurance, 4771 (56.4%) had public insurance and 465 (5.5%) lack insurance. Blacks were more likely than whites to be < 65 (66% vs. 58%, p < .0001), female (21% vs 17%, p < .014), a current cigarette smoker (39% vs. 25%, p < .0001), lack insurance (7.5% vs 5%), or have public insurance (66% vs. 56%) as opposed to private insurance (26.5% vs 39%) (p < .0001). However, advanced stage did not differ between blacks and whites (89% vs. 87%). Multivariable logistic regression showed a lower likelihood of advanced stage disease diagnosis among females (OR 0.52 [95% CI:0.44-0.60], p< 0.001), patients > 65 (OR 0.62 [95% CI:0.52-0.72], p< 0.001), and urban residents (OR 0.71 [95% CI: 0.52-0.98], p=0.037). Black race (OR=1.173, p=0.28), smoking, Hispanic ethnicity, and insurance status were not associated with advanced disease.

Conclusions: In our study population, blacks were more likely than whites to lack private insurance and be a current cigarette smoker. Advanced OPC stage was more likely among males, those < 65, and rural residents. However, advanced stage was unaffected by black race, cigarette smoking, and insurance status. These insights will aid in the design of future public health interventions.
