Introduction: Though women have made great strides regarding representation in the field of Facial Plastic and Reconstructive Surgery, their presence among academic advancement continues to be underreported.

Methods: A bibliometric analysis of the most cited articles from eighteen high impact Facial Plastic and Reconstructive surgery journals was conducted using the Elsevier Scopus database. Female representation among first, senior, and corresponding authors was assessed using the US Social Security Administration Baby Names Database. Chi square analysis was used to compare the relative number of female authors between decades and nationalities.

Results: The top 100 articles (published between 1951-2013) had the most total number of citations between 1997-2001. 70% of the articles were published by U.S. institutions. From the query and further analysis, the gender of 99% of first authors, 93% of senior authors, and 95% of corresponding authors was identified. Of which, 12% of first authors, 13% of senior authors, and 11% of corresponding authors were women. In U.S articles, women accounted for 11% of first authors and 15% of senior authors, while in international articles, women accounted for 14% of first authors and 7% of senior authors. No statistically significant difference in the relative number of first, senior, and corresponding female authors was observed between decades of publication and article nationality (domestic vs international).

Conclusions: Despite the increasing number of female facial plastic surgery trainees, the relative representation of female academic contributions continues to trail. Additional efforts are needed to support and highlight achievements of female facial plastic surgeons.
